Ben's Trismus

Ben, an 8 year old American Pit Bull Terrier, was initially evaluated by our dentist, Dr. Huff for a chief complaint of trismus (unable to open his mouth) and oral pain. Upon anesthetized examination, Ben was found to have a large mass arising from the back portion of his right lower jaw (mandible). A computed tomographic (CT) scan of Ben’s jaw was utilized to determine the extent and severity of the tumor. The CT scan revealed extensive destruction of Ben’s bone (termed osteolysis) at the level of his temporomandibular (TMJ) joint, which explains why he was unable to open his mouth.

After completion of the CT scan, a surgical biospy of the mass was performed. The biopsy confirmed a mandibular fibrosarcoma, which is the second most common tumor to arise from within the oral cavity in dogs.

After return of the biopsy results, Ben’s owner met with our oncologist, Dr. Scope, to discuss therapeutic options for Ben's tumor. A decision was made to attempt surgical removal of the mass through a radical surgical procedure termed a mandibulectomy. During this procedure, the tumor and an associated portion of the jaw are removed. The tissues are then reconstructed to allow for both a functional and cosmetic closure.

Two weeks after diagnosis, Ben returned to the hospital for evaluation by our surgeon, Dr. Cavanaugh. Ben’s procedure was performed the same morning and he was allowed to go home from the hospital the following evening. Dogs tolerate oral surgery extremely well, even in cases where large portions of the jaw need to be removed. Most dogs will begin eating within 24-48 hours after a mandibulectomy.

Ben has continued to do well in the postoperative period and normal, pain-free function has been restored to his mouth.

Ben’s case is a great example of how it is often necessary to utilize a team of veterinary specialists in order to effectively manage a complex case. This type of collaboration ensures that each component of the patient’s medical care is managed by the best equipped individual.
